Such care requires teams of community stakeholders working together with the same end goal in mind: to improve the health of the people in our community so that there is opportunity for growth and advancement of society as a whole. These teams of stakeholders should include individual health practitioners, hospitals and clinics, public health departments, wellness providers and organizations, government agencies, academia, employers, schools, to name a few.
